OVERVIEW

Butterfly Stages of Life Care values our team members and offers competitive benefits to eligible employees. Because we operate on an as-needed, assignment-based staffing model, benefit eligibility is based on hours worked and length of service as outlined below.

ELIGIBILITY REQUIREMENTS

Benefits eligibility is determined by:

  1. Average weekly hours worked over the specified timeframe
  2. Length of continuous service with the company
  3. Active status on our roster (accepting assignments and remaining in good standing)

Important: Hours are calculated based on actual hours worked, not hours available or on-call.

BENEFITS TIER STRUCTURE

TIER 1: After 90 Days of Service

Eligible employees working 35+ hours per week

Paid Time Off (PTO)

  • Accrual: Earn 1 PTO day after maintaining 35+ hours/week for 90 consecutive days
  • Ongoing Accrual: Earn 1 additional PTO day every 60 days while maintaining 35+ hours/week average
  • Usage: PTO must be requested at least 7 days in advance when possible
  • Carryover: Up to 3 days may carry over to the next calendar year
  • Payout: PTO is paid out upon resigning with a 2–3 week notice or termination

How PTO is Calculated:

  • Hours are averaged over rolling 90-day and 60-day periods
  • If average hours drop below 35/week, PTO accrual pauses until eligibility is regained
  • Previously earned PTO is not forfeited due to temporary hour reductions
